Olm sits in the middle of its chamber and is fought from either side — the head and both claws are separate targets, and the claws have to be dealt with before the head takes meaningful damage.
Loot comes from the Chest at the end, paid out on your contribution to the raid rather than on the killing blow. Uniques are broadcast to the world and logged in the collection log.
